The American Association of State Highway and Transportation Officials creates the basic standards and offers recommendations that most states tend to adopt. There are many guidelines for how to incorporate bicycle lanes and signage.

They sell their Guide for the Development of Bicycle Facilities for $45 in the bookstore at their website. But you can download it for free as a pdf from CommunityMobility.org.

OREGON BICYCLE AND PEDESTRIAN PLAN
We consider the Oregon Bicycle and Pedestrian Program to be a model for how to do this right.
Their Bicycle and Pedestrian Plan, created in 1995, is available by mail for free to Oregon residents, for $10 for others, or you can download a pdf version for free. The whole plan is a big file, but it is also available broken down in smaller files, if you prefer.
